Default 2011 Chevy 3500 cutaway cube van shake

I have a 2011 Chevy 3500 cutaway cube van that has a terrible shake to it. When driving down the road between 40 and 60 mph, the van will shake violently until you come to a complete stop. I have taken it to local dealers and shops for the past year, and no one can diagnose the problem. I had the whole front end replaced. I have had the box company tell me it is mechanical, while the repair shops tell me its structural. It just seams like no one wants to work it. If anyone has any insight, please let me know. Bad tire. Needing balancing? Bent or damage wheel? Some foreign matter caked onto the inside of a wheel.....

I have had the tires rotated, balanced, changed and alined 4 or 5 times this year.

A few things you could check into... Could be a bad tire. The tire may balance perfectly but if there is a flat spot or something not visible to the naked eye, it could be your problem. (I had this problem with a car) The box is mounted to the frame with U bolts. I have had them tightened before, but never caused a shake. But worth checking. Bent rim.. probably not, a bent rim would not balance. How tall is the box? Does it have a wind deflector above the cab? Any damages to that? Last resort find a good commercial truck center/dealer. Someone who will deals with all aspects rather than pointing the finger.

Not sure what my thoughts are worth but if it is a dually maybe one of the rear tires is flat or out of balance. Also read you had the front end gone through but I must ask what is the condition of the lower and upper ball joints?
