Default No heat on driver side 04 Suburban Z71?

I have no heat on driver's side but passenger side works perfectly. The service manual shows three actuators; one left, one right and one called the mode. I located the left (driver side) actuator under the dash and was able to pull the lever completely to the bottom, which is presumably the "full heat" position. The control was in the highest heat position, but when I started the engine, the lever went all of the way back to the top, and continued to blow cold air.

Questions: 1) Does anyone know which of the three actuators controls the driver's side heat? 2) Does the fact that the left actuator returns to the "cold" position indicate that it may be in the "control" rather than in the actuator?

Thanks for any assistance.
