Default 2500 Brake pad change. Ints and recomm.

I need to change the pads on my 2000 Sub 2500 4x4. I looked online for directions and only found some on the AutoZone website. I was wondering if anyone may know of a site that has some better instructions. Also I did do some research and some have said that the rotors are pressed on while others say you have to use a bolt to separate them from the hub. Which is correct? I am also wondering what is a recommended pad to use, Ceramic or Semi Metalic. I may not have to do the rotors yet as they still look pretty good. Any help and recommendations are appreciated.

A Chiltons manual has some pretty good step by step info for a brake change. But I haven't changed brakes on a 4wd in a very long time. As for Ceramic, or Semi metalic. I have no preference.
