1991 with 305, intermitently runs rough

I have a 1991 Chevy Caprice wagon with the 305. It has 131000 miles and generally runs very well. Occasionally if it sits for a couple days, the car will run progressively worse as the engine warms. To the point that it may stall upon acceleration. I can shut it off, let it cool for a 1/2 hour and restart. The engine will run fine. What's up????

When was the last time it was fully tuned?
Maybe a sticky EGR???
My 91 has a habit of the engine dying if I'm backing up....more than 5-6 seconds...starts right back up.....FREEKIN thing!! LOL!!

Thanks RixZ-28. I did have it tuned and it seemed to solve 95% of the problem. Every so often it still sputters and misfires a bit. With the warmer weather I've been driving it more frequently and it is not the issue it once was.
