high idle and code 22

my 1995 3500 has high idle and code 22. Replaced TPS, throttle body gasket, IAC, and no obvious vacuum leaks to be found. When i give it gas while driving the Check engine light goes out but comes back on when i let off the peddle to shift or come to a stop. Any suggestions?

Code 22 means the TPS returned a sense voltage less than 0.16V. The TPS receives 5V reference voltage and returns some intermediate voltage depending on the throttle opening. With a multimeter verify it's getting 5V and returning >= 0.5V with key on, engine off:
Terminal A, GRY wire, 5V Ref, from PCM
Terminal B, BLK wire, ground ref, from PCM
Terminal C, DK BLU wire, TPS Sens, to PCM, should be > 0.16V

Can you read higher voltages by manually opening the throttle valve? Terminal C should put out from 0.5V to as high as 4.5V at WOT. Is it possible that the TPS is misaligned on the throttle shaft?
