P/S pump shaft broken again!

I just replaced the PS pump in my 04 Tahoe 42k. The dam steering/brakes went out again. The shaft turns fine and there is no metal in the oil. I assume the shaft broke again. What gives? No sign of trouble except for loss of steering/brakes. I bought a DELCO reman hoping it would last.
I disassembled the last pump, that's why I know it was broke. HELP!!

Any one of these items listed below can lead to a second P/S pump failure:
