Default 2006 HD2500 "Delay at take off" (??)

I'm looking at a 2006 Chevy HD2500 fleet truck for sale with 203,000 miles, 6.0 liter, 2WD, regular cab. Has a goose neck hitch. They want $4995 for it. Its actually pretty straight and clean. Interior good, ice cold SC...but one little thing bugs the crap out of me. From a dead stop, such as a red light, I let off the brake and hit the gas, and there is a slight delay. The engine revs, then the truck "engages" for lack of better term, and it then moves, and after that all is normal; runs through the gears. The one thing I want to mention in addition to this delay, which by the way happens in forward AND reverse, is that there is a "whining" sound from some unknown location that changes pitch with the speed of the wheels, not the motor. My father-in-law told me, from his many cattle trailer pulling experiences, that the diff could be damaged from pulling heavy loads. (Goose Neck I mentioned earlier) Is this issue something serious? Or should I buy the truck at this stupid low price and cross the bridge when I come to it?
