Default Intermediate steering shaft

I have a 2000 express 3500 with 5.7. Got a clunk in the steering and some weaving on the highway In the past 6 weeks, I have replaced shocks, ball joints, tie rod ends, controll arm bushings, idler arms and mounts and it still has the clunk. Steering gear was replaced 3 years ago so I do not suspect it.

Has anyone replaced the intermediate steering shaft and if so, can you give me some directions on how to remove and replace it?
