Default possible suspension or alignment problems?

Hi,
I'm driving an 07 Impala, I think there may be a suspension problem or an alignment problem. When I go over small bumps at slow speeds i hear a clunk/shutter in the rear of the car, also when I'm driving at highway speeds the left rear wheel shakes. The final problem is that there is uneven tread wear in both rear tires (the inside tread is wearing much quicker than the rest of the tread).

What could be any possible problems that would cause these symptoms, I believe that they are all related since they are all in the rear and mainly localized to the left rear. Could this be a possible suspension problem (broken or worn parts) or could it be that I need an alignment (I had an alignment done a few months ago and after that is when i started noticing the clunking).

Any help is greatly appreciated,
