Default Slip yoke seal

Hey, I recently discovered that grease was coming out of both ends of my slip yoke on my Driveshaft after pulling my transmission. I have dismantled it and discovered that the seal had started to brake into pieces under the cap, which I presume is the reason why it was leaking grease. I have spent a number of hours looking for a replacement seal or at least a part number, but so far I have not come up with a serviceable seal.
Can anyone point me in the direction of a seal or a seal alternative?

4.3 Engine
​4L60e Transmission
3 joint driveshaft
LS Extended cab
Driveshaft: Three Rivers 26039018. Cst 039691 UAW Local 2093
