sounds silly, but check your motor oil. by cold starts do you mean cold days, or engine has cooled down to outside temp?
sounds like your motor oil is draining down b/c you don't have enough motor oil so your truck is taking a few minutes to lubricate

it's not uncommon for a high mileage truck to slowly burn motor oil

is it a loud knock, or more of a ticking?
ticking= could be stuck lifters.
knocking=possibly main bearing of oil pump
did this knocking/leaking start after a recent oil change? or has it been happening for a while now?

either way, sounds like a severe case of "dry start": could have negative consequences in terms of engine life
