Default '04 tahoe Blend Actuator

My wifes tahoe is having ac problems. When running the air, it will switch from front vents to defrost, blow cold air on one side and hot on the other etc. It seems to be only the front that is having the problems. The rear air seems to work fine.

The tahoe has the manual dual climate control (not digital).

I was told it was probably the blend door actuator.

Couple of questions:

1.) Where can i purchase one of these? What is part number. The only ones i see are for the auto temp control vehicles.

2.) Where is the thing located? Do i need any special tools to fix? Does it need to be reprogrammed?

was told it is actually a mode actuator that i need.

Anybody help???
