Default Hard Brake lines on 2001 Suburban

I saw your post to McGyver9 on 4-27-13 about replacing his hard brake lines. I too have to do this and cannot figure out which bolts to undue on the drivers side and where to place the two pole jacks mentioned. How far do I need to raise body up from frame? Can I use an 8 ton bottle jack and 2.5 ton hydraulic jack instead as I do not have any pole jacks. I have the stainless kit but not sure how to snake all lines in. Any help is greatly appreciated.
Mine is 2001 suburban LT 2500 4x4.
