Okay got a response from the GM-truck forum the kits includes:

1 remote, replacement RKE module, antenna and manual, no install instructions (dealer has to get them).

Okay. Just confirming what in2pro informed us of. I just got off of the phone with my GM guy. Yes the kit is
1 remote, replacement RKE module, antenna. There is no part number for a remote fob only. GM told him if you want another fob you have to order the entire kit. Pricing is $200 for the kit, but I'm getting it for $150. I was quoted $196 for the install which is 2 hours of labor, which their GM book says this install takes. Realistically it should only take about 45 minutes.

GM part direct sells the kit for ~$200, it might be worthwhile to order the kit and install it your self and then take it to the dealer for programming.
Just a thought if your handicapible... I also understand the FOB is somewhat larger then the standard FOB...

There isn't too much to take pictures of as the module will be behind a panel in the cargo area. The antenna will be put in the back cargo window on the drivers side. Other than that there will just be the fob. Again not too much to really take pictures of.

When they showed it on Trucks, it appeared to be about 50% bigger than the regular fob.
