Default What could it be ???

I have second truck, 2000 suburban Ls when I crank it it runs fine I will let it run for 10/15 minutes but still when I pull off it take off slow. It seems once it warmed up from driving 6/8 minutes it pulls off fine also with like jerks like you feel the transmission shifting.Only at start offat the begging. Then the truck seems to have no issue once you drive a while. Is it the transmission or something easy to fix. This only does it when the truck sits for 30/minuts or a hour. Please help thxs
