Default Noise in 1st gear only?

I have a 92 Silverado 2wd. When i take off, my truck makes a noise until it switches over to 2nd gear. It's like a grinding noise, like a chain dragging on the ground noise. Then my trans has trouble switching to 2nd gear also.

Any idea?

auto or manual, if there is a grinding noise in any trans, it usually means bad news, rebuild usually in order

As 2wd-K5 said... once a transmission starts making "grinding" noises it usually means overhaul time.

How many miles? Are you aware of any previous transmission work being done?

If I were you, I'd drop the pan IMMEDIATELY and empty the fluid into a clean (preferably new) oil pan and start sifting through the oil with a small magnet and also with your fingers. (If you rub your fingers together and it feels gritty, that's metal) If you see/feel small metal shavings I would STOP driving the car right away because you could be causing even more damage. But even if you don't find metal shavings in the oil, they may be stuck in the filter (which is rare) and in any case automatic transmissions are a pain in the **** and once again, once they start "grinding" it's generally time to pull it.

The truck has 258,000 miles on it, yeah I know, it's a lot, but the motor still runs good. It has a rebuilt tranny and had rear end work done on it. I had someone tell me it could be the rear end.

I would be inclined to stay with transmission according to your diagnosis. I mean, if it was the rear end (or U-Joint) you would hear it constantly... or your would hear it in certain ranges. (IE: 5-10mph, then nothing until say 30-35mph) If it goes away once you've shifted out of 1st, and then everything is fine after that I just can't see how it would be rear end.
