Default Gearing quandry

Currently I have a 1988 C3500 with 454 and TH400 tranny dually.

I use to own a 1962 International p/up.

The low "Granny" gear was amazing in that truck..I could put in 1st and put cinder-block on gas pedal and get out of truck and walk along side of it (if I slowed down so truck could keep up).

Also like the International, this truck seems to sound like it needs another higher gear or 2 maby.

So...
Is there a way I can get the best of both worlds and get my "granny" gear back and have a lower RPM top gear for highway driving and fuel savings?

I do alot of open desert driving ( the need for a "granny") but coming from northern Calif and going to S. Cal or Arizona, the fuel bill is killin me.

Could swap in a 5-speed manual with a granny. Should be relatively plentiful, and its not to difficult of a swap. With a 454, good luck improving the fuel mileage no matter what you do.
