1999 Suburban Heater Control Valve

Unhappy 1999 Suburban Heater Control Valve

Hey everyone I need help, I replced my lower intake gasket on my 99 Suburban. As I was adding water to the radiator I noticed water running out by the firewall. At first I was freaking out but I found out it was not the work I had done. The water was coming out one of the hoses going to the Heater Control Valve. Here's my problem when I grabbed the hose man all the hoses came off. it was dry rotted so bad the plastic just crumbled. Of course I want to put it back together right. you would think it would be obvious but to me it's not.
Can anyone help me by giving me a diagram the shows which hoses go where and which hoses connect to the tee,s that are installed into the Heater Contol Valve.
The main problem I am having is there is 2 different size hoses and ports.
With it just crumbling apart I was not able to see how they were all installed.
PLEASE HELP SOMEONE!!!!!
