A few 2003 1500 HD questions..

I am new to the forum, but have lurked some in the past. I appreciate the wealth of knowledge that is available.
I've got a few issues with my truck and also need some basic advice and info.
First the truck is a 2003 Silverado 1500 HD 4X4 with approx. 91,000 miles. As the mileage is now getting up there, I figured it was about time to change the plugs and wires. My basic problem is, I'm not quite sure how to get the boots off of the plugs. I've twisted them side to side and pulled fairly hard, but, I'm afraid to pull too hard for fear I will cause damage. Is there a method to this? Or do I just need to pull harder? Frankly, I'm not sure I can get a hold on the well enough to pull them off. Also, does the metal shroud around come off with the boot or is it fixed in place?
Second, my truck has recently started idling rough at low RPMs, in fact it died last night for the first time. I know changing plugs and wires may help, but, what else should I be looking at?
And lastly, I have a terrible rattle when idling that appears to be coming from a loose exhaust joint. However, there is a metal "spacer" that seems like it would keep me from tighting the clamp any tighter around that joint. Any ideas on how to approach/fix this?

just pull harder they may break but if your changing them it dosnt matter the metal shields i think come out with the plugs as far as the rough idle possible fuel filter pressure regulator or even pump exhaust rattle not too sure i gutted all mine
