I wouldn't go with a used part.
O'Reilly and Autozone should have Four Seasons or Murray 35015.

Either way, you'll need to swap the squirrel cage over to the new unit.

last time I used rock auto, I bought a coolant reservoir and it didn't fit. then I bought a tail light and it didn't fit. looked back at both of the product pages and they both said that they fit for a 2004 chevy suburban 1500. so I no longer use rock auto. go to auto zone or o'Reillys. but I mainly use eBay motors and have yet to have a problem.

Ok thanks for the suggestions…looks like my local autozone had them in stock. I went to pull the motor this morning and can’t seem to get it out. Haven’t messed with it a ton, but thought I’d see if there is a trick to it. It mounts from the top with 3 screws, (not on the side like the 2000+ models). Can’t seem to fit it out between the plastic duct it’s mounted in and the metal fender. Is there a way to remove the fan from the motor before you remove the motor?

Fan assemblies typically mount to the motor shaft with an allen-head set screw.
Post some real good photos of yours from several angles; there's always a way, and replacement of a blower fan motor would not be an "unexpected repair" when engineering these.

Hey I did manage to get it. Fairly simple, just popped the two bolts holding the top of the entire ductwork to the vehicle, and was able to pry it out enough to slide the fan assembly out!
