Default 2004 Tahoe Z71 Heater Hose Assembly

hey guys my question is about the hose that is connected to the water pump and has a T connector that has a smaller hose going to the fire wall and a larger hose going to the coolant overflow tank. The T connector has dry rotted or something so it has a small hole in it so it is leaking. What is the best way to fix it? Can I get a new "T" connector or should I get the whole heater hose assembly?
Thanks!!

I believe it usually comes as an assembly but if you shop around enough you might find the connector/fitting

if its dry rotted personally i'd do the whole assembly
