Default Blower Motor Resistor

Hello,
I have a 2004 Suburban LT 4x4 the A/C stop blowing for the previouse owner, so he replaced the resistor, but it would only blow on high, could not be ajusted, so i replaced it with this

Now it does not blow at all, im not sure if its bad out of the box or I got the wrong one. I have checked the fuses, I have the digital control w/ auto, it also has the center console controls for the rear A/C. Does anyone have the OEM part number.

I am about to replace the same thing for the 3rd time in 5 years. The original resistor in my '01 Suburban lasted 13 years, so my plan is to go right to the dealer and pay the extra cash and change it out for hopefully the last time. They are not fun to get to.
