Default Rich mixture

Hi All.
1996 2.2 automatic Cavalier coupe. Gas smell in exhaust, black plugs, terrible gas milage. What are the most common causes for rich mixture? What should I look for?

Injector stiuck open. Pull the plugs. The plug with the stuck injector will be a mess, the other three will look nornmal

If an injector has been dumping fuel into a cylinder a lot of that gas will get past the rings and into the oil, diluting it. Pull the dipstick, if it smells of gas, change the oil.

Other possibility: temp sensor telling the Engine control module that the engine is cold, resulting in rich fueling.
