Default AC - Rear evaporator replacement

I have two leaks in my AC system. The biggest one is on the connection in the rear evaporator core, where the connection is.
I ordered a new evaporator, and am planning on replacing the evaporator along with teh hoses that go to it.
1. Does anyone have a link or a video with instructions how to correctly replace the rear evaporator core?
2. HOw would I find out the model, or part number of the hoses I need to replace? So I can order them too.
